Primers flipping or falling out of cup

I am having an issue with my 550 in which my small primers are "flipping" sideways in the primer cup or are being flipped/thrown completely out of the cup. This is occurring about 10-15 times for every 100 primers. If I pull my bar slowly, so that I can try and watch the process, the primer is picked up from the tube, the primer is right side up and flat up until the point that the cup start to reach the plate. I then lose sight and it is kind of hard to see what is happening.
I do know that this occurs more often the faster I start to reload. If I go really slow I can tend to make it not happen.
Any ideas?

The flipping is likely due to the primer cup not lining up with the hole in the platform that the primer comes up through. Since it lines up properly when you slow down, I suspect either the underside of the primer slide is dirty, or the short end of the operating rod that moves the slide back and forth is dry and sticking in the bracket clamped around the primer shield.
Use alcohol and a scotchbrite pad to scrub the underside of the slide and top of the frame. Then do the same to the short end at the top of the operating rod, except also grease the short end of the rod that goes into the hole in the bracket.

This may be obvious but make sure you have the proper primer bar and proper primer tube in the primer mag. I had this same thing happen to me before I realized I had the large primer bar in while using small primers. I switch everything over except the primet bar when I got interupted. Now I check both bar and primer tube before I start a session.
